1. The requestor will generate interest in a class and collect a $100 deposit from each interested person.+
2. The requestor will provide a location for the class~.
3. When deposits have been received from 15 people (not including the requestor-who will get a scholarship), the requestor will contact CSIMT.
4. Together the requestor and CSIMT will select a class date, which is a minimum of 60 days out from the date of 15 minimum confirmed deposits.
5. CSIMT will notify the students of the selected date. Students will be given two weeks to notify CSIMT if they cannot make that date. After that two weeks, CSIMT will then process their $100 deposit (ie-run the credit card through, cash a check, etc.), At this time, the CSIMT cancellation policies goes into effect – see below.
6. The class date/location will go on the public CSIMT calendar.
7. If there are less than 12 paying students in the class, within 22 days of the class date, CSIMT reserves the right to cancel or postpone the class.
8. CSIMT will administer^ the class as it does all CSIMT classes.
9. The requestor will provide facilitation* for the class.
10. The requestor/facility will receive 1 full scholarship, to the class, for every 10 paying students~.
+Please be aware of Registration/Cancellation/Discount Policies:
A $100 non-refundable, non-transferrable deposit is due upon registration. If CSIMT cancels a class, a full refund will be given. Please note that the balance will be billed to the credit card, that the deposit was charged to, 45 days before the class. If you do not want your credit card billed, it is your responsibility to make other arrangements 60 days before the class. Payments by check must be received 45 days before the class or your registration is not guaranteed. We reserve the right to change our fee.
There is a 15% discount for 3 or more students, from the same facility, that register for the same class, at the same time. Requests for this discount must be made upon registration, otherwise it will not be given.
There is a 50% discount for full-time students and students repeating a class. Requests for this discount must be made upon registration, otherwise it will not be given. You must supply proof. This does not include students of the CTSIMT. Requests for this discount must be made upon registration, otherwise it will not be given.
~If the facility is your employer, you/the facility, will receive one scholarship, to this or any CSIMT course, per ten paying students. If you have to "find" a facility, the facility will receive one scholarship, to this or any CSIMT course, per ten paying students. With every additional 10 students, CSIMT will give the requestor one scholarship to this or any CSIMT course. CSIMT'spreference is to utilize a facility (PT office, hospital, school). If this is not possible, hotel details will be worked out between CSIMT and the requestor.
